Welcome to Advocate Care Solutions, a Home and Community-Based Services (HCBS) agency dedicated to providing Housing Stabilization Services to individuals facing housing challenges. Our goal is to prevent homelessness and promote housing stability by offering tailored support and resources.

Eligibility

Requirements

✅18 years old or older


✅Must be on Medical Assistance (MA)


✅Must have a documented disability or disabling condition


✅Homelessness or at risk of being homeless

SERVICES

Providing Support and Resources

Housing Counseling

Individuals and families experiencing housing instability will receive personalized counseling sessions. Trained counselors will assess needs, provide housing information, offer budgeting help, and assist in creating action plans for housing stability.

Rental Assistance Programs

Financial aid available for eligible individuals and families includes rent subsidies, utility assistance, security deposit help, and other support to relieve housing cost burdens.

Eviction Prevention 

To prevent eviction and homelessness, the program offers legal aid, mediation, and negotiation to tenants, working with landlords to reach mutually beneficial resolutions and prevent displacements.

Case Management 

Participants are assigned dedicated case managers who offer continuous support, advocacy, and referrals for essential services to address housing instability and root causes.

Housing Navigation

Navigators help individuals and families find housing by offering guidance on preferences, budget, needs, applications, inspections, and lease negotiations.

About Us

Advocate Care Solutions, an employee-owned cooperative founded in Minnesota in 2024, focuses on creating safe housing services. We believe in the fundamental right to live in a secure environment for overall well-being, emphasizing the importance of safe housing for personal growth, stability, and mental health.

Have a documented disability or disabling condition, defined as one of the following
A person who is aged, blind or has a disability as described under Title Il of the Social Security Act.
A person with an injury or illness that is expected to cause extended or long-term incapacitation
A person with a developmental disability (or related condition) or mental illness
A person with a mental health condition, substance use disorder or physical injury that required a residential level of care and who is now in the process of transitioning to the community
A person with a substance use disorder and is enrolled in a treatment program or is on a waiting list for a treatment program
Are you experiencing housing instability, evidenced by one of the following risk factors:
Homeless. An individual or family is considered homeless when they lack a fixed, adequate nighttime residence
Currently transitioning, or has recently transitioned, from an institution or licensed or registered setting (registered housing with services facility, board and lodge, boarding care, adult foster care or community residential setting, hospital, Intermediate Care Facility for persons with Developmental Disabilities (ICF/DD), intensive residential treatment services, the MN Hospital..
Are you at risk of homelessness? An individual or family is at risk of homelessness
At risk of institutionalization - meets an institutional level of care/eligible for the following waivers
